I have a few resume questions?

hello alls i just had a resume questioin. i wasnt sure where this belonged at but i will ask anyway. what should i do if the places that i had worked at went out of business/burnt down? the one place that i worked at they have a new location should i just use that address? also, it has been about eh 8-9 years since i have worked, do i still need to do a resume? the place that i am applayin for is a place called aramark and for the consession stand do i still need to do a cover letter and resume?? plus when i was working it was before i got married, so do i just put my maiden name in (parenthiesis)
